Join NYC REACH as we partner with Healthix to discuss best practices for achieving and sustaining Patient-Centered Medical Home (PCMH) workflows using Healthix’s health information exchange tools.

The NYS PCMH program requires that all primary care practices seeking PCMH recognition in New York State connect to a Qualified Entity (QE), also known as a Regional Health Information Organization (RHIO). Healthix is the largest QE/RHIO in New York State. Practices and health centers that connect to Healthix are able to use an array of health information exchange tools in Healthix’s portal, including alerts when patients are admitted or discharged from the hospital and seen in the ED, access to medical record information from other healthcare providers, and more.

These important Healthix tools support a number of PCMH workflows. In fact, Healthix tools streamline key PCMH processes that previously, many practices find to be among the most time-consuming. Whether you are live on Healthix, currently implementing Healthix, or considering joining, join us to learn practical tips and workflows to use Healthix’s functionality to achieve and sustain PCMH.
