Healthix is partnering with CHCANYS and seven FQHCs to bring patient-reported social determinants of health (SDoH) information into HIE data. This effort is supported through the Healthix Innovation Fund and is slated to begin later this year.
Social data, such as unemployment and unstable housing, will be captured through the Protocol for Responding to and Assessing Patient Assets, Risks and Experiences (PRAPARE) assessment tool and stored via Azara Healthcare, a data warehouse and reporting tool licensed by CHCANYS. With patient consent, Healthix will allow care team members across different organizations to access critical SDoH information through the Healthix portal to inform more appropriate patient care and interventions.
In collaboration with CHCANYS, Healthix will solicit feedback from FQHCs, the Healthix Clinical Population Medicine Committee and other Healthix users on how to optimally integrate SDoH into Healthix products and services.
