Recently appointed Healthix Board Member, Joseph Lamantia, Executive Director of Northwell Health Solutions and VP, Population Health Management, is leading the new Healthix Population Health Committee. The committee, which kicks off on June 14, brings together a group from diverse organizations – payers, primary care physicians, community based organizations so they can help advance value-based care and address opportunities from different perspectives.
“Payment for value-based care is slowly taking hold in New York. The change contemplates moving providers from a single visit fee-for-service model, towards viewing the patient as a whole and paying based upon performance (quality and outcomes). Providers and care managers are beginning to work more collaboratively across the continuum of care to improve key measures that are linked to performance. Practically speaking, this means following up with patients beyond the four walls of a hospital or physician practice, often weeks or months following an episode of care to help ensure patients have what they need to manage their condition, including things like stable housing, transportation for doctors’ appointments and adequate nutrition. In population health management we are focused on subsets of the population – particularly high and rising risk patients. For those that are identified as high risk and in need for a “heavy touch,” data and health information exchanges are used to deliver alerts that help us intervene at the point of care, as well as telephonic engagement when they are back in their home. This translates overall into better care for the patient.”
